The Federal Judicial system. They have forms that you fill out they have the instructions on how to fill them out.

The "paralegal" who prepared your forms should correct his or her own errors at no additional charge to you. I am concerned about what you were told. First of all, paralegals are NOT allowed to give legal advice in California, and that includes advising you what forms to file with your bankruptcy case. Only attorneys can legally offer that legal advice. Second of all, what you were told is incorrect. If a creditor was omitted from your bankruptcy schedules, you need to amend Schedule F (if it was an unsecured creditor) OR Schedule B (if it was a secured creditor) OR schedule E (if it is a Priority unsecured creditor) AND you need to amend the creditor matrix (the item you were presumably told to amend). I would be concerned about the "paralegal" who is doing this work for you also -- but since (presumably) you already paid him or her and he or she did not do what you paid for, go back and have them prepare the corrected forms (HOPEFULLY correctly -- they do NOT "just" need to amend the creditor matrix -- they must amend the appropriate SCHEDULE also and FILE it in your bankruptcy case. You should be able to find the forms you need here: http://www.uscourts.gov/bankruptcycourts/prose.html
